Community Outreach Coordinator: Key Responsibilities

The ideal candidate will have Nursing Licensure (LPN minimum) or MSW credentials plus 2 years of leadership experience in Public Health.
This is not a work from home position and is based in Roanoke, VA.
Essential Responsibilities:
- Maternal Health Outcomes

- Ensure that high quality, culturally tailored social support is promoted as the standard of practice staff to clients, stakeholders, and the community.
- Carry out outreach contacts with local community partners to ensure updated information is shared related to services provided by UBB, i.e., program, classes, support.
- Work to ensure program is meeting enrollment goals and outcomes as set by program directors by completing intakes for needed services.
- Community Engagement
- Establish and maintain a referral process with the local hospitals, clinics, physicians, and community agencies.
- Perform street level community outreach in target communities to promote specific programs.
- Maintain and develop local outreach & public awareness materials (brochures, posters, flyers, etc.) to promote UBB services
- Attend Community Health Fairs, as appropriate, to promote UBB Services.
- Ensure reporting is completed and submitting all required documentation in a timely manner. Submit month end report to Director by the 5th of each month.
- Provide education to targeted communities about the importance of healthy life styles, prenatal care, having a medical home, as well as other important health promotion messages.
- UBB Orientation & Support
- Manage relationships with and provide coaching to out of area outreach specialists to ensure compliancewith program practices, values, and standards
- Maintain timely communications with referral site managers and staff via email, social media, and phone
- Maintain electronic records for all services provided
- Monitor enrollment services to ensure compliance with program standards and policies
- Case Management
- Conduct presentations to potential clients; orient clients to referral process; performs preliminary client screening including suitability and eligibility; provide program information
- Manage the referral and intake process for the outreach program in community-based, hospital, and clinical settings
- Support new and existing patients by scheduling health visits at an appropriate health center or in home.
- Collect client information for records, including eligibility documents, assessment forms, and intake documents
- Follow up on referrals to ensure proper referral processing and to assess outcomes
- Create and/or edit flyers according to established events and as directed
- Increase awareness regarding content to post on Facebook and share via email about the programs, ourevents, and other information of interest to our community
- Assist with process and systems improvement, including implementing new tools and procedures for workflow efficiency.
- Maintain caseload of 15 home visiting clients monthly
- Data & Compliance Management
- Maintain organized and complete intake files for new enrollees
- Provide weekly and monthly reports.
- Conduct data entry of services and analyze data for continuous quality improvement.
- Maintain detailed record of activities to regularly track impact of outreach efforts and to facilitate appropriate reports to funders and government agencies.
- Communicate with Director regarding compliance issues.
- Report program trends to the management team.
- General Staff Responsibilities:
- Participate in staff meetings, conferences, training sessions and workshops as assigned.
- Demonstrate familiarity with employment policies, performance standards, work plan and objectives of program.
- Maintain congenial and respectful relations with staff, children, families and community.
- Maintain confidentiality regarding staff, program, and participant information.
- Maintain objectives and professional standards.
- Improve self-skills and education.
- Perform any other work-related duties as requested by Director.
- Be present at work in order to provide consistency of services.
- Be a contributory team member in a positive/productive manner.
- Demonstrate commitment to mission, values, and policies in the performance of daily duties.

- Qualifications:
- Nursing Licensure (LPN) or MSW plus 2 years of leadership experience in Public Health
- Two years’ experience working with Maternal/Child Health is recommended.
- Prior experience in community organizing and/or performing outreach at the community level, preferred.
- Ability to work a flexible schedule; some evenings and weekends will be required.
- Current physical examination, immunizations including TDAP, and TB screen documentation upon hire
- Desire to work with children and their families
- Adequate means of transportation with ability to travel within the area and urban/rural cities up to a 45 mile radius
- Ability to establish relationship of trust and respect with staff, families, and children.
- Ability to plan, coordinate, and organize work projects
- Strong facilitation and presentation skills
- Strong communication and writing skills
- Experience collaborating with a variety of staff
- Strong attention to detail
- Experience in working with communities of color and strong racial, disability, gender and economic justice framework
- Commitment to anti-oppression and de-colonization work with a focus on racial equity and reproductive justice, specific to birth justice
- Passion for and commitment to the mission of UBB
- Aptitude for solving problems with creativity and resourcefulness
- Ability to develop positive relationships with clients, doulas, home visitors, providers, staff
- Intermediate Proficiency with Microsoft Office Applications, especially Excel and Word, Google
