Care Coordinator Supervisor

Urban Baby Beginnings is seeking a Full Time Care Coordinator Supervisor

with our program and reports directly to the Community Clinical Integration Manager. The goal of this position is to support the integration of UBB’s program with community-based partners  at Urban Baby Beginnings, effectively addressing social determinants of health and improving healthcare access and outcomes for pregnant and postpartum families.  In addition to supervisory responsibilities, this position includes a 30% job requirement for Home Visits and Care Coordination, which involves conducting in-home assessments and collaborating with healthcare professionals to develop and implement care plans tailored to individual patient needs. This position is based in Richmond, VA and requires travel and will support the operations between 5 maternal hubs in Virginia.  This is not a work from home position.

Key Responsibilities:

  1. Home Visits (30%): Conduct in-home and clinical assessments, ensuring patient comfort and safety.
  2. Care Coordination (30%): Collaborate with healthcare professionals to develop and implement tailored care plans.
  3. Patient Education: Provide education on perinatal and infant health.
  4. Health Promotion: Promote preventive care and healthy living through educational programs and workshops.
  5. Documentation: Maintain accurate patient records, documenting all assessments and interventions.
  6. Advocacy: Advocate for patients’ needs to ensure high-quality care.
  7. Community Engagement: Build relationships with community organizations to foster collaboration and promote health equity.
  8. Strategy Development: Collaborate to develop and refine the maternal hub home visiting integration strategy.
  9. Coordination and Communication: Ensure consistent communication among nurse home visitors, families, and pediatric providers.
  10. Efficiency and Sustainability: Identify opportunities to improve the efficiency and sustainability of integrated care models.
  11. Data Analysis: Collect and analyze healthcare data to track outcomes and measure the impact.
  12. Quality Improvement: Monitor service quality and implement evidence-based practices.
  13. Supervision: Provide supervision and support to staff, ensuring adherence to best practices and program goals.

Essential Responsibilities:

  1. Maternal Health Outcomes:

– Promote culturally tailored social support and integration with clinical systems.

– Collaborate to ensure seamless integration of services.

– Provide 1:1 support through home visits, group facilitation, and enhanced case management.

– Conduct nurse visits, offering education on maternal and child health.

– Support outreach efforts to increase awareness of UBB’s programs.

– Educate and advocate within the community, emphasizing healthcare integration.

– Assist with data collection and documentation of outreach activities.

– Connect individuals to services through EHR platform.

  1. UBB Orientation & Support:

– Complete assigned home visiting online training.

– Complete all paperwork within 24 hours of assignment.

– Maintain organized care coordination and home visit files.

– Keep electronic records for all services provided.

– Participate in weekly reflective sessions with the interdisciplinary team.

  1. Case Management (30%):

– Be accessible at hospitals/clinics and main offices to conduct evaluations and screenings.

– Support medical staff and interdisciplinary teams to ensure continuity of care.

– Assess client needs and develop client-centered care plans.

– Work with a team to help clients achieve self-sufficiency and stability.

– Initiate contact with clients within 24 hours of referral.

– Make referrals to ancillary agencies and community resources.

– Provide education, referrals, and follow-up based on client needs.

– Coordinate care plans with short- and long-term goals.

– Advocate for clients and liaise between programs.

– Participate in events to boost enrollment and advocacy efforts.

– Maintain organized and complete client files.

– Research and provide community resources for clients.

  1. Data & Compliance Management
  • Maintain organized and complete client documentation files within 24 hours of visit
  • Work closely with the coordinator to ensure compliance.
  • Track and provide accurate screening tools for each client, within our program timelines for screening.
  • Communicate with staff regarding compliance issues.
  • Report program trends to the team.

General Staff Responsibilities:

  • Participate in staff meetings, conferences, training sessions and workshops as assigned.
  • Demonstrate familiarity with employment policies, performance standards, work plan and objectives of program.
  • Maintain congenial and respectful relations with staff, children, families and community.
  • Maintain confidentiality regarding staff, program, and participant information.
  • Maintain objectives and professional standards.
  • Improve self-skills and education.
  • Perform any other work-related duties as requested by Coordinator or Director.
  • Be present at work in order to provide consistency of services.
  • Be a contributory team member in a positive/productive manner.
  • Demonstrate commitment to mission, values, and policies in the performance of daily duties.
  • Other duties as assigned

QUALIFICATIONS TO APPLY

Education/Experience:

State Licensure or Certification as RN, QMHP, MPH, MSW or related role in the perinatal health space, Master’s Degree, Minimum of 3 years of Maternal Health and Public Health experience with Case Management and Leadership preferred.

License:

RN and MSW must have active and unencumbered license to practice as a Registered Nurse or Medical Social Worker. QMPH must have valid certification  Valid VA driver’s license with auto insurance coverage required.

  1. Current physical examination, immunizations including TDAP, and TB screen documentation upon hire & signed by medical provider
  2. Desire to work with children and their families
  3. Adequate means of transportation with ability and willingness to travel within the throughout the State of Virginia
  4. Active and unencumbered driver’s license + automobile insurance
  5. Background and sex offender check clearance
  6. Ability to establish relationship of trust and respect with staff, families, and children.
  7. Two years or more of case management experience (resources, referrals, support planning, service management, advocacy support) certification/training
  8. Ability to plan, coordinate, and organize work projects
  9. Strong facilitation and presentation skills
  10. Strong communication and writing skills
  11. Experience collaborating with a variety of staff
  12. Strong attention to detail
  13. Experience in working with communities of color and strong racial, disability, gender and economic justice framework
  14. Commitment to anti-oppression and de-colonization work with a focus on racial equity and reproductive justice, specific to birth justice
  15. Passion for and commitment to the mission of UBB
  16. Aptitude for solving problems with creativity and resourcefulness
  17. Ability to develop positive relationships with clients, home visitors, providers, staff
  18. Proficiency with Microsoft Office Applications, especially Excel and Word, Google
