Pregnancy & Early Parenting Care Coordination Services

Through Urban Baby Beginnings® and our ReByrth™ virtual platform, families may receive personalized health and wellness support based on program eligibility. Our care coordinators help connect you to the services and care that best meet your needs, including:

• Pregnancy and postpartum home visits in the comfort of your home, UBB's maternal health hub, or location of your choosing

• Doula Concierge support from trained Doula practitioners

• Holistic education and coaching on topics such as nutrition, mental health, breastfeeding, safe sleep, and newborn care

• Access to group classes and support circles, including childbirth education, breastfeeding, postpartum healing, and infant care

• One-on-one lactation support through our partnership with ReByrth™, a telehealth platform for moms

• Diaper Bank and essential baby care resources for enrolled UBB members and families in crisis

• Connections to trusted community-based resources and referrals

• Early childhood support and development programming for children ages 0 to 2

This program supports pregnant and postpartum individuals through early parenting, up to their child’s 24th month. Some services may be subject to eligibility or availability based on location.
